Unfourtunatly Scott didnt get the operation and now Sara was pregnant. The fact that Scott got Sara pregnant instead of his own wife would cause Grace to hate Sara

Scott had forgotten that he left the door unlocked. Even if Sara got out, Colin had no memory of her, so he wouldn't recognize her if she approached him.

Soon Sara gave birth to a baby boy but knew she wouldn't be able to look after it so she put the baby infront of an orphanage and knocked the door and hid in the bushes
